Our Services

Caring Heart Family Nurse Practitioner is your local primary care provider in Palm Coast Florida.

We serve patients in Palm Coast, Bunnell, Daytona Beach, Ormond Beach, St Augustine, and Flagler Beach. We offer home visits for homebound patients and telehealth. Our clinic or office visit is open Monday to Friday from 9am to 10 pm by appointment only.

Imaging studies such as chest x-ray, ultrasound, echocardiogram, or 12 lead EKG can be done at the comfort of your home except for self-pay patients. We also provide blood draw at home with our independent phlebotomist.

OurGoal

One of the CHFNP's goals is to help increase access to medical care for Florida residents.

We accept Medicare, Medicaid, Sunshine Health, United Health PPO, BCBS PPO, and Tricare Select. We offer home visit and Tele-health for homebound patients.
