Caring Heart Family Nurse Practitioner (CHFNP) is a Family Practice started and owned by Evangeline Diaz Butler, a Board Certified Family Nurse Practitioner in Palm Coast, FL
CHFNP works collaboratively with other healthcare professionals to provide optimum care to our patients in Palm Coast, Flagler Beach and Daytona Beach. We also extend our affordable health care services to Florida patients through Telehealth.
For now, CHFNP accepts Medicare, Medicaid, BCBS, United Health, and Self-Pay patients. We offer group discounts for home visits and FL student nurses in Palm Coast and Flagler Beach. We also provide free Telehealth screening for a scheduled appointment. Spanish-speaking Nurses are available if you need one.
We are a group of Florida Registered Nurses, Physical Therapists, Mental Health Specialists, Home Health and Wound Care Nurses.
